Example conversation
You: We need a medication policy for our designated centre for older people—ordering, storage, administration, MAR charts, and errors.
Agent: I’ll use the NSSBH reference (safe care, medication) and HIQA guidance for designated centres. I’ll draft a medication policy covering: ordering and supply; storage and security; administration and who may administer; self-administration where permitted; MAR and recording; errors and incidents; disposal; training; and medication review. I’ll link to incident reporting and care plans.

Medication policy excerpt
Administration — Medicines are administered by [e.g. qualified nurses / trained staff]. The MAR chart is completed at the time of administration. Any refusal or omission is recorded with reason. Self-administration is only where risk-assessed and documented in the care plan.
Errors and incidents — Any medication error or near miss is reported using the incident reporting procedure. The resident is monitored and the prescriber/GP informed as appropriate. Incidents are reviewed to prevent recurrence.
