When we disclose — We disclose when an incident has resulted in harm or potential harm to a patient. Disclosure is timely, honest, and compassionate. The treating clinician (or designated person) leads the conversation, with support available for the patient/family and for staff.
What we say — We acknowledge what happened, apologise where appropriate, explain what we are doing to support the patient and prevent recurrence, and say how we will keep the patient/family informed. We document the disclosure and any follow-up.
